Plot Summary: The story opens with a radio announcement about the out-of-control crime in Tokyo. Among-st this chaos, a gorgeous woman hires a private detective from the City Hunter group to find her sister, Karumi, who is missing. Disappointed with police action, she decides to hire Ryo Saeba (Akira Koizumi), a famous private detective and one-time Special Forces soldier who is not just deadly in combat and brilliant in tactics but also has a strange weakness for gorgeous women.
Ryo and Hideyuki quickly find Karumi, who is cornered by Yakuza. The Yakuza attacks, and Ryo is able to defeat them with his combat skills, but Karumi misunderstands the situation and considers Ryo a pervert, so she runs away. Makimura collects a weird liquid dropped by Karumi, and keeps it hidden from Ryo in order to investigate it.
Their investigation introduces Seiko Nogami, a law-abiding policewoman and one of Makimura’s former colleagues. Seiko warns City Hunter not to get involved in Karumi’s case. Nonetheless, Ryo and Makimura are hot on their trail, and their hunt ends in a final battle where Makimura is mortally wounded by a truck driver induced by a strange serum who shows up and attacks them. Makimura gathers Ryo in its life and passes him the makeup serum before dying, requesting him to take care of his sister Kaori.
A month later, she asks Ryo to help her investigate her brother’s death. At first reluctant, Ryo is pulled back into the fray when he discovers the serum has connections to an old drug called angel dust, which turns individuals into mindless death machines. After 20 very long years, Seiko and Ryo discover that this drug's reappearance is linked to the Union Syndicate, headed by the enigmatic Kunio Kono.
Relentless in getting to the bottom of this mystery, Kaori tails Ryo to a cosplay festival, where Karumi's again singled out by the Union Syndicate. Karumi ends up getting kidnapped despite Ryo's attempts. After they track Karumi to a Union Syndicate laboratory, Ryo and Kaori discover the Syndicate's intention to create zombie-like soldiers with the serum.
In a climactic showdown in Tokyo, Ryo and Kaori invade the Syndicate’s headquarters. They then confront a man named Kono, who discloses Kaori’s true parentage: her biological father was a victim of the angel dust project, murdered by Makimura’s dad. This revelation strengthens Kaori’s resolve. She confronts Kono with Ryo's help, but the Union Syndicate ends up killing him to keep their secrets.
The tale ends with the Union Syndicate broken down and peace returning. Kaori becomes Ryo’s new partner and blackmails him into accepting her by threatening to throw out his porn collection if he doesn’t. Karumi resumes her lifestyle as a cosplayer, Ryo continues his lifestyle as a detective.
